耳後部有茎皮弁にて外耳道再建した両側性外耳道扁平上皮癌の1例

はじめに

　聴器悪性腫瘍は，その発生部位によって耳介腫瘍，外耳道腫瘍，中耳腫瘍に分類されるが，いずれも稀な疾患である。その発生頻度は報告により差があるが，人口100万人あたり1〜6人と報告されている^1,2）。また，両側の外耳道悪性腫瘍はさらに稀であり，われわれが渉猟した範囲では過去に10編の報告がなされているのみである^3-12）。それらの報告のなかで，両側同時発症の症例は3編で報告されており^3,6,7），また，上咽頭悪性腫瘍などに対する放射線照射の既往が外耳道悪性腫瘍の発生に影響した可能性が否定できない症例が2編で報告されている^4,8）。外耳道悪性腫瘍は組織学的には扁平上皮癌が大部分を占め，外科的治療を中心に放射線治療や化学療法も行われている。今回，われわれは極めて稀な両側外耳道扁平上皮癌症例を経験したので，文献的考察を含めて報告する。
