口唇口蓋裂に対する外科的アプローチ

POINT

●口唇口蓋裂は最も頻度の高い先天性外表奇形の1つであり，変形に伴う整容的・機能的障害に対して，成長に合わせて段階的な手術治療が行われる。

●口唇裂手術の主な目的は整容の改善である。短縮した口唇を延長して左右対称で自然な外観を得るために種々の術式が考案されている。

●口蓋裂手術の主な目的は鼻咽腔閉鎖機能の獲得である。口蓋の長さを延長し，軟口蓋において左右に断裂した筋束を確実に再建することで機能予後が改善する。

●口唇口蓋裂の治療においては，形成外科をはじめ，歯科・口腔外科，耳鼻咽喉科，小児科など複数の診療部門がかかわる集学的治療が必須であり，患者の成長が完了するまで継続的に行われる。
