当直医へのコール

●耳性頭蓋内合併症には，髄膜炎，脳（S状）静脈洞血栓症，頭蓋内膿瘍などがあるが，その原因や初期の病像は小児と成人では異なる^1）。

●小児例では髄膜炎でみられるような頭痛，嘔気，嘔吐，発熱などの全身症状が主体であるのに対して，成人例では脳梗塞様の意識レベルの低下や神経脱落症状が前面に出る場合が多い。

●救急外来において頭蓋内病変が認められた際に耳鼻咽喉科当直医がコールされるのは，①中耳・内耳の症状があるときと，②画像検査などで聴器に異常が疑われたときである。

●頭蓋内病変の診断がついていない場合もあることから，中耳・内耳疾患では常に髄膜炎や脳梗塞様の頭蓋内病変を示唆する所見を見落とさないことが大切である。
