耳管の解剖と生理

POINT

●耳管は頭蓋底深部を走行する管状構造物で，全長の約1/3が骨部，約2/3が軟骨部，その境界が峡部に分類される。軟骨部で耳管の生理機能が発揮される。

●耳管管腔の周囲には多くの構造物があり，全体として複雑な構造になっている。特に口蓋帆張筋は耳管開大筋であり，耳管軟骨外側板を牽引して耳管を開大させる。

●耳管の生理機能は，古くから防御，排泄，換気といわれているが，それらにより中耳腔の正常な機能が保たれており，その破綻は深刻な中耳病変をきたす。

●耳管の通過性は受動的耳管開大能であり，これに対して嚥下時に生じる通過性の変化は能動的耳管開大能という。
