高齢めまい患者を診断する際のポイント—加齢性平衡障害の診断基準を中心に

POINT

●高齢者では体平衡維持に関わるすべての感覚器官（末梢・中枢前庭系，視覚系，体性感覚系），中枢での感覚統合，および出力系である運動器官の機能低下から，平衡障害をきたす。

●良性発作性頭位めまい症（BPPV）は高齢者に比較的多くみられるめまい疾患である。

●presby-vestibulopathy（加齢性平衡障害）は，持続性の平衡障害を主症状とし軽度〜中等度の両側前庭機能低下を認める疾患として，2019年にBárány Societyから診断基準が提起された。

●presby-vestibulopathyの診断基準が提起されたことで，今後，高齢者のめまいの原因疾患の分布が変化すると考えられる。
