小児中耳真珠腫—経外耳道的内視鏡下耳科手術（TEES）の適応と手技の実際

POINT

●中耳真珠腫は進行すると難聴だけでなく，めまいや顔面神経麻痺などの重篤な合併症を生じる疾患であり，QOLを保つためにも早期に適切な手術治療が必要とされる。

●経外耳道的内視鏡下耳科手術（TEES）はすべての手術操作を細径内視鏡を用いて外耳道から行う手術手技であり，耳後切開や術後の圧迫固定が不要で，小児においても早期から日常生活への復帰が可能な低侵襲手術である。

●中耳真珠腫に対するTEESの適応決定には，術前CTに加え，MRI拡散強調画像を用いた進展範囲の評価が重要である。鼓室から乳突洞までの真珠腫であれば，TEESでの手術治療が可能である。

●TEESでは広角な視野をもつ内視鏡を対象に接近させ，さらに斜視鏡を組み合わせて観察することで，死角の少ない明瞭で拡大した術野を得ることができる。
