米国での真珠腫手術

Point

●米国における中耳手術の発展は，William Houseをはじめとする近代のneurotologist／otologistの功績が大きい。

●人種間での中耳真珠腫の発生率は異なり，白人，黒人，ヒスパニック，黄色人種の順に多い。

●手術アプローチはcanal wall up（CWU），もしくはcanal wall down（CWD）が基本であるが，CWD後にcanal wall reconstruction（CWR），もしくはmastoid obliterationを行う手術法もある。

●CWRを行う場合は外耳道後壁を一時的に外し，CWDと同じ視野で手術操作が可能であり，その後，外耳道後壁再建により外耳道の自然な自浄能を保つことができるメリットがある。
