術中出血のコントロール—上手な局所麻酔，術野のマネジメント

POINT

●視野と操作性の確保が手術操作の鍵となる。

●術前の消炎は出血が減少して術野が良好になる。

●手術は術野を内視鏡画面正中に位置させて肩の力を抜いて行う。

●全身の血圧を下げることにより術中出血が減少する。

●局所アドレナリンの使用は，術野での出血量を減少させる。

●眼窩壁・頭蓋底・前篩骨動脈を先に同定して副損傷を回避する。
