ヘルペス属ウイルスと顔面神経麻痺

POINT

●顔面神経麻痺全体の約7割をヘルペス性神経障害が占め，なかでもBell麻痺とHunt症候群は十分な理解が必要な疾患である。

●ステロイドと抗ウイルス薬が治療の主軸となるが，いずれも早期治療介入が重要である。

●初診時に3主症状のすべてが揃うHunt症候群は少ないため，zoster sine herpete（ZSH）の可能性も念頭におき治療を行う。

●水痘ワクチンは帯状疱疹の発症抑制にも明らかに有効であり，Hunt症候群の罹患頻度減少が期待されている。今後，発症に関する水痘・帯状疱疹ウイルスワクチンの介入コホート研究の必要性が増してきている。
