耳痛・耳漏—耳性頭蓋内合併症／悪性外耳道炎（頭蓋底骨髄炎）／ANCA関連血管炎性中耳炎／外耳道悪性腫瘍／髄液漏／側頭骨悪性腫瘍／巨細胞性動脈炎

POINT

●外来診療において，耳痛・耳漏を伴うcommon diseaseのなかから，危険な疾患を見逃さないように努める義務がある。

●悪性外耳道炎（頭蓋底骨髄炎），外耳道悪性腫瘍，耳性頭蓋内合併症などは，早期診断が重要である。

●診断確定のための組織生検は積極的に検討すべきであるが，組織生検を先行すべきか，画像精査を先行すべきかを十分に検討する必要がある。

●50歳以上の高齢者の耳痛で，耳内所見が正常であれば，巨細胞性動脈炎も鑑別に挙げる必要がある。
