POINT
●典型的な多形腺腫のMRI像は，分葉状で境界明瞭，T2強調画像（T2WI）で被膜様構造を持ち，内部がT2WIで高信号，T1強調画像（T1WI）で低信号である。
●ただし，多形腺腫の内部は，①軟骨腫／粘液腫様組織，②線維状組織，③細胞成分の豊富さ，によってT2WIの信号強度は変化する。高信号を示す（①が多い）のは全体の約3/4で，②，③で構成される部分が多いと低〜等信号となる。
●多形腺腫以外でT2WIが高信号を示すことが多いものとしては，良性では神経鞘腫が挙げられ，腺房細胞癌，腺様囊胞癌などの低〜中悪性度癌は比較的高信号が多く，一部に被膜様構造がみられる。
●ワルチン腫瘍は境界明瞭な類円形のことが多く，被膜様構造は欠くことが多い。T1WI，T2WIのいずれも低信号であるが，囊胞性変化などにより内部は不均一なことが多く，囊胞部分は高信号のことが多いが，粘性な場合は低〜等信号となる。
●シェーグレン症候群では，主に耳下腺でT1WI，T2WIのいずれも内部不均一な像として描出され，T2WIや脂肪抑制画像で高信号な斑点状の陰影がみられるが，このような背景に加えて腫瘤性病変を持つ場合は，悪性リンパ腫を想定しておく必要がある。
