顔面神経麻痺を呈した頭蓋底骨髄炎の1例

はじめに

　典型的な頭蓋底骨髄炎は，高齢糖尿病患者に発症した緑膿菌による悪性外耳道炎を契機として頭蓋底へ炎症が波及したもので，時に致命的となりうる疾患である^1,2）。近年は抗菌薬の進歩により，以前と比べ予後は改善しているが，脳神経症状は予後不良因子とする報告もある^3）。今回われわれは，顔面神経麻痺を呈した頭蓋底骨髄炎の症例を経験した。手術ならびに十分な抗菌薬投与を行い，顔面神経麻痺は残存したものの，耳痛など他症状は消退し，CTでも後頭骨に新たな骨梁形成を認めたので報告する。
