POINT

●外側咽頭後リンパ節（RPLN）はルビエールリンパ節とも呼ばれ，前方で咽頭収縮筋，後方で椎骨前筋膜を境とする咽頭後間隙に存在し，特に上咽頭がん，中咽頭側壁・後壁がん，下咽頭後壁がんで転移が好発するといわれている。

●下咽頭喉頭全摘術と同時にRPLN郭清を施行する場合，術野を広く展開することが可能であり，重要臓器を温存しながら比較的容易かつ迅速に郭清組織を摘出できる。

●局所進行下咽頭・喉頭がん症例の根治手術に際しては，原発巣の亜部位や多発リンパ節転移の有無にかかわらず，同側RPLN郭清は考慮されるべきであると考える。
