頭頸部外科患者の術後ICU管理

POINT

●頭頸部腫瘍患者は基礎疾患を持つフレイル患者が多く，遊離皮弁を用いた再建手術が多いため，術後の気道・呼吸器合併症のリスクが高い。

●術後のICU入室は必須ではないが，喫煙歴，人工呼吸器の必要性，両側頸部郭清の術式，APACHE Ⅱスコア10点以上，大量輸血などの患者側リスクと，ICUの病床数や病棟のマンパワーなどの施設環境を考慮した適応判断が必要である。

●頭頸部腫瘍に対する遊離皮弁移植手術後のICU管理は，遊離皮弁の管理と全身管理がメインとなる。

●皮弁管理において術後の低血圧は避ける必要があるが，過剰輸液は避けるべきで昇圧薬の使用が有用である。

●全身管理のポイントは呼吸不全とせん妄の予防で，多職種によるチーム医療が有用である。
