POINT
●日本での経鼻的持続陽圧(CPAP)治療の保険診療上の適応は,自覚症状が強く,日常生活に支障がある症例で,AHI≧40であれば検査施設外睡眠検査(OCST)による診断でも導入できる。
●CPAP治療のアドヒアランス向上のキーポイントは患者教育と副作用対策である。
●患者教育では,閉塞性睡眠時無呼吸症(OSA)発症の原因,治療の必要性,他の治療でなくCPAP治療が適応になる理由を説明し,納得させることが重要である。
●代表的な臨床上の副作用は,インターフェース関連や治療圧関連が主であり,特に耐圧性の問題については耳鼻咽喉科医師による鼻呼吸障害への対処が重要である。
