下気道真菌症

POINT

●下気道に定着して病態を形成しうる真菌は，分生子のサイズや至適発芽温度などの条件を満たすアスペルギルス・フミガーツスなど一部の真菌に限定される。

●宿主の気道病変・免疫状態によって，アレルギー性気管支肺アスペルギルス症，慢性肺アスペルギルス症，侵襲性肺アスペルギルス症など多彩な病態をきたす。

●病態によって，診断に適したモダリティ（血清反応，ガラクトマンナン抗原，PCRなど）を使い分けることが重要である。

●アゾール系経口抗真菌薬を中心に，病態，真菌の種類，薬剤感受性，他臓器機能などに応じて治療を選択する。
