仮道への涙管チューブ留置によりcheese wiringを生じた症例

はじめに

　cheese wiringとは，涙管チューブ挿入後に涙点と涙小管の周囲の軟部組織が，チーズをワイヤーで切ったようにチューブによって切断されることを指す^1）。眼角部の疼痛，霧視，鼻汁のほか，流涙をきたすことがあるが，無症状の症例も報告されている。涙管チューブ挿入後にcheese wiringをきたした症例の報告は4例と少なく，いずれも鼻腔内視鏡下で涙管チューブを抜去されている^2,3）。

　今回われわれは，仮道への涙管チューブ留置によるcheese wiringを生じ，同時に流涙が再発した症例に対して内視鏡下涙囊鼻腔吻合術（endoscopic dacryocystorhinostomy：EN-DCR）を施行し，良好な術後経過を得た症例を経験したため，報告する。
