耳下腺癌の頸部リンパ節転移に対する診断と治療—耳下腺癌274例の自験例から

Summary
●耳下腺癌に対する頸部郭清術の適応や郭清範囲，特にcN0症例に対する予防的頸部郭清術（END）について多くの議論がある。
●耳下腺癌全274例中，臨床的リンパ節転移陽性（cN＋）は61例（22.3％），病理学的リンパ節転移陽性（pN＋）は71例（25.9％）であった。T分類別のpN＋は，T1は38例中2例（5.3％），T2は111例中12例（10.8％），T3は37例中16例（43.2％），T4は88例中41例（46.6％）であった。悪性度別では低／中悪性が156例中7例（4.5％），高悪性が118中64例（54.2％）であった。
●107例に対してENDを施行したところ，潜在的リンパ節転移は16例（15.0％）に認められた。
●転移部位をみたとき，転移頻度が高い順に，レベルⅡ，耳下腺周囲，レベルⅢ，レベルⅣ，レベルⅤの順であった。
●疾患特異的5年生存率をN分類でみたとき，N0が90.0％，N1が72.2％，N2が44.7％であり，転移陽性症例の予後は不良であることから，耳下腺癌におけるリンパ節転移の制御が重要であることがわかった。ENDは組織学的高悪性症例が適応と考えた。
