急性中耳炎・急性乳突蜂巣炎—鼓膜切開の適応と抗菌薬の選択

POINT

●急性中耳炎は，耳痛，啼泣・不機嫌，発熱などの臨床症状が急性に発症し，鼓膜の膨隆，耳漏／中耳貯留液，発赤などの鼓膜所見が認められた場合に診断される。

●臨床症状と鼓膜所見から軽症／中等症／重症に分類し，その重症度に応じて抗菌薬で治療する。

●一次選択薬抗菌薬群として高用量AMPCやCVA/AMPC，二次選択抗菌薬群としてCVA/AMPCに加えてCDTR-PI，TFLX，TBPM-PIが推奨される。

●重症例および中等症例のなかでは鼓膜の膨隆が強く耳痛・発熱が高度の場合には，鼓膜切開も検討される。

●急性乳突蜂巣炎の有無に注意し，診断した場合には入院治療を行う。
