鼻副鼻腔内反性乳頭腫34例の臨床的検討

はじめに

　鼻副鼻腔に発生する内反性乳頭腫（inverted papilloma：以下IP）は，鼻副鼻腔腫瘍の0.7〜7％を占める良性の腫瘍である^1）。好発年齢は50〜60代，男女比は3：1と男性に多く，年間10万人あたり0.6人の発生と報告されている^2,3）。手術の再発率が比較的高く（2.3〜28％），悪性腫瘍が合併することもあるため（5〜15％），局所の徹底的な切除が推奨されている^4,5）。分類法としてはKrouseによるstaging system^6）（以下Krouse分類）が提唱されており，手術法の選択などに広く用いられている。以前は手術法として外側鼻切開手術による上顎部分切除が一般的であったが，内視鏡やナビゲーションシステムなどの手術支援機器の発達により，近年では内視鏡下鼻副鼻腔手術（endoscopic sinus surgery：ESS）を始め，endoscopic medial maxillectomy（EMM）などの侵襲の少ない術式が用いられることが多い^7,8）。またIPは原則として良性であるため，下鼻甲介と鼻涙管を温存したendoscopic modified medial maxillectomy（EMMM）も術式として選択可能である^9）。今回われわれは，当科で過去10年における鼻副鼻腔IPの手術症例34例について臨床的に検討し，手術方法や予後について考察を行った。
