摘出せずに開放術を選択した蝶形骨洞動脈瘤様骨囊胞例

はじめに

　動脈瘤様骨囊胞は1942年にJaffeとLichtensteinによって報告された良性の病変で，20歳以下に多く^1），女性優位に発生する^2）。腸骨や骨盤，脊椎などに好発するが^3），蝶形骨洞に発生するのは非常に稀である。病理組織像で骨囊胞内に血液を含む空洞を形成し，壁に線維成分やマクロファージ，巨細胞などが認められることから^4），特発性に生じるもののほか，外傷や腫瘍による血管異常によって発生する二次性のものがある^5）。二次性の場合，原因病変には巨細胞腫，骨芽細胞腫，血管腫，軟骨芽細胞腫などがある^5）。術前診断に画像検査が有用で，MRIのT1強調画像で低信号，T2強調画像で高信号を示す^6）。CTでは局所的な骨浸潤と側壁の菲薄化が認められる^7）。治療法は手術が第一選択で，全摘出が望ましいが，摘出困難な部位に対しては掻爬術が行われることが多い。今回，蝶形骨洞に発生し，蝶形骨洞後壁が菲薄化していて掻爬も困難なため，蝶形骨洞の開放のみを施行した結果，経過良好であった例を報告し，手術治療法の選択について考察する。
