唾液腺癌に対する薬物療法

POINT

●唾液腺癌はきわめて多様な病理組織像を呈し，多数の腫瘍型があるため，それぞれの生物学的特徴を考慮して薬物治療の方針を検討する。

●腫瘍型とバイオマーカーに基づく個別化治療の開発，臓器横断的（tumor-agnostic）な治療開発，従来の抗がん剤治療の研究により，唾液腺癌に対する薬物治療の選択枝が増えた。

●HER2陽性SDCでは抗HER2療法，AR陽性SDCでは抗アンドロゲン療法，NTRK融合遺伝子陽性分泌癌では抗NTRK療法，腫瘍変異量高値（TMB-H）またはPD-L1陽性唾液腺癌症例ではペムブロリズマブ，BRAF V600E遺伝子変異症例ではダブラフェニブ＋トラメチニブの投与を検討する。

●腫瘍型にかかわらず，プラチナ製剤＋タキサン系抗がん剤の併用療法の有用性が報告されている。
