脳卒中をはじめとする脳損傷後の機能回復には,損傷を受けた脳内の神経ネットワークの再構成,機能代償が関与していることが近年の機能画像を用いた研究によって明らかになってきている1,2).歩行障害は脳損傷後の患者において高頻度に見られ,その回復は日常生活動作自立の観点からも重要であるが,背景にある脳内機構についてはいまだ不明な部分が多い.

 本稿では,ヒトにおける歩行機能の回復に関連する神経ネットワークの変化について,筆者らの施設で行った歩行時の脳活動測定の結果などを中心に,リハビリテーション介入に対する効果も含めて概説する.
