認知運動療法の可能性と限界

21世紀は「脳の世紀」といわれる．脳科学の領域における研究はこの10～20年で飛躍的に進歩し，知覚や運動のメカニズムの解明にとどまらず，これまで自然科学が立ち入らなかった意識や心の問題にまで研究対象を広げてきている．米国では1990年に『脳の10年（Decade of the Brain）』を定め，精力的に研究が進められており，欧州でも同様の活動が始まっている．また，わが国においても「脳の世紀推進会議^1）」，「理化学研究所・脳科学総合研究センター^2）」，「脳科学の先端的研究（先端脳）^3）」などの研究機関が主導し，一流の科学者が脳の機能の解明に挑戦している．

　このような脳科学の研究がわれわれセラピストにもたらした大きな功績は，1）脳の機能についての理解が進んだこと，2）これまで常識とされていた脳の機能が見直されてきたこと，3）学習によって脳の可塑性に働きかけができることが解明されたことであろう．1970年代にイタリアでアイデアが出された認知運動療法（esercizio terapeutico conoscitivo：ETC）は，このような脳科学の知見を重要な基礎の一つに位置付け，現在も展開し続けている運動機能再教育のための治療体系である．本稿では，ETCの本質と脳科学を基礎とした展開の過程，およびその可能性と限界の考え方について解説する．
