計算課題が脳血管障害者の歩行動作に与える影響

脳血管障害者の理学療法の主要な目的のひとつに「独立した歩行能力の再獲得」がある．歩行の運動制御に影響を与える因子として路面や障害物などの外的環境はもちろんであるが，思考や会話など内的な環境も影響を与える重要な因子である．歩行中の思考や会話は歩行に対する注意を分散させることにより実現すると言われる．Woollacottら^1）は，注意は個人の情報処理能力として定義されるとし，歩行中の会話といった2つの課題を同時に行う際に両者の課題が個人の能力以上を要求するならば一方あるいは両方の行動が拙劣になるとしている．

　注意の分散や認知機能が立位・歩行に与える影響についての研究は，年代による影響の大きさの変化^2）や，パーキンソン病など神経系の疾患による影響^3～5），認知機能の低下を伴うアルツハイマー病における同時課題遂行能力の検証^6），変形性膝関節症患者の認知課題と重心動揺の変化を検討しているもの^7）などが行われている．脳血管障害者に関する研究ではBowenら^8）が60歳以上で10m以上杖歩行が可能な退院後の在宅生活者を対象に，会話が歩行の速度とバランスに与える影響についての検討を行っている．また，Haggardら^9）は，数種類の認知課題と歩行の2つの課題を行ったときの両者の干渉の影響を健常者と脳損傷者を対象に行っている．
