日本における心臓移植の現状と理学療法―米国における実際とアウトカムも含めて

心臓移植は，不治の末期的状態にある方に脳死者からのドナー心を胸郭内に埋め込むため，一生薬物療法が必要となるが，その心臓によって救命，延命を図るのみならず社会復帰をも目指す治療法である．本稿では，日本における心臓移植手術の現状と移植後の理学療法について，移植先進国の米国における理学療法の実際とアウトカムを含めて詳述する．

これまでの心臓移植

　1997年10月16日に「臓器移植法」が施行され，1999年2月に脳死ドナーによる心臓移植第1例が実施され，日本でも脳死での臓器提供による移植が可能になった．南アフリカで世界最初の心臓移植が行われてから約30年，アメリカではすでに年間4,500件の臓器提供が行われており，他の先進国でも一般の医療として定着している^1）．最近ではアジア各国でも多くの心臓移植が行われるようになり，2000年9月までに台湾で354件，韓国170件，タイで158件の心臓移植が行われている^2）．
