脳卒中治療ガイドラインと歩行障害の理学療法

はじめに

　情報技術の進歩によるインターネット普及は，短時間で大量の情報を得られる環境を私たちに提供した．また，医療界においては，PubMedやPEDro，Cochrane Libraryといった信頼できるデータベースの整備が進み，その検索により最新のエビデンスを容易に得ることが可能となった．さらに，PEDro scoreなどのデータベースによる研究の信頼性や妥当性評価は，信頼性の高い大規模なRCTs（randomized controlled trials：無作為化比較試験）へと研究者を誘導し，多くの良質な研究が日々登録されている．

　さて，1999年に日本脳卒中学会および日本神経治療学会において脳卒中ガイドライン作成の必要性が提起され，脳卒中関連5学会（日本脳卒中学会，日本神経治療学会，日本神経学会，日本脳神経外科学会，日本リハビリテーション医学会）によってエビデンスに基づいたガイドラインの作成が進めらた．そして，2004年4月に「脳卒中治療ガイドライン2004」として脳卒中治療の指針が示された．このガイドラインに示されたエビデンスは2002年頃までの内容であり，その後，多くのsystematic review（系統的レビュー）^1～5）やRCTsが発表されている．

　本稿では，歩行障害の理学療法を中心にガイドラインの概要を説明し，その後発表されたエビデンスを概説するとともに，課題について述べる．
