人工股関節再置換術後に坐骨神経由来の歩行時痛を呈した症例に対する理学療法経験

はじめに

　近年，人工関節の普及，改良，手術手技の向上により変形性関節症における人工関節の除痛効果は非常に有効とされ，術後に歩行時痛が発症し，支障を来す割合は極めて少ない．しかし近年，人工関節置換術の適応が拡大し，術後，新たに坐骨神経障害の発症とともに歩行障害を認めたとする報告が散見されるようになった．その原因としては，脚の延長に伴う坐骨神経の過緊張を原因とする脚延長説^1～3）と，術中の操作に併発し坐骨神経障害が発生する術中操作説の2つに分類されている^4～6）．

　今回，人工股関節再置換術後の脚延長によって発生した坐骨神経障害により，著明な歩行時痛を呈した症例を経験したので，その経過とともに神経症状発現機序ならびにわれわれが実施した理学療法について，文献的考察を加えて報告する．なお，患者本人には投稿に関する同意を得た．
