認知リハビリテーションの最前線

はじめに

　認知リハビリテーション（以下，認知リハ）とは，脳損傷に起因する認知障害を治療的介入によって回復に導こうとするものである^1）．認知リハでは，認知障害のタイプや重症度，障害されている機能と残存機能を把握し，介入方法を検討する．この際，患者の呈する認知障害はどのようなメカニズムに基づいて回復または代償できるのか，選択する介入方法は脳内のいかなるメカニズムを援助しようとしているのかについて検討し，神経系の回復メカニズム論や残存機能に基づいた戦略決定・方法選択を行うことが重要である^1）．さらに，その効果は，検査課題の成績向上と日常生活上の改善という2つのレベルで判定する^2）．例えば前頭葉損傷患者が，日常生活において自発性の欠如，計画性のなさ，衝動的行動などを呈していた場合，この障害の背景にある機能障害（例えば遂行機能障害，注意障害など）を精査し介入することで，機能障害の改善のみならず，日常生活上の問題を軽減することを目的とする（図）．

　認知リハの対象は，失語・失行・失認といった古典的高次脳機能障害に加え，注意障害，記憶障害，遂行機能障害，視空間障害，情動機能障害，社会的行動障害などへ適応の幅を広げている^3,4）．これらの障害は，脳血管障害に併発することも少なくなく，認知リハの観点から援助方法を学ぶことは，理学療法の円滑な進行と効果の向上に寄与する可能性が高いと考えられる．
