疼痛を有する対象者の包括的理学療法

はじめに

　理学療法士は，リハビリテーション医療全般にわたる臨床場面において，様々な疼痛を抱える人々のケアに関与している．疼痛は，症状の時間経過により急性痛（acute pain）と慢性痛（chronic pain）に分類され，急性痛は，6か月以内に消えるのが一般的であるのに対して，慢性痛は，6か月以上経過しても治らない痛みとされている^1）．急性痛は，疼痛の発生源である組織傷害部が明確であり，それに対する積極的な医学的アプローチを施せば治癒する可能性が高い．一方，慢性痛は，疼痛の症状が長期化するため，身体だけでなく不安や苦悩など精神面に対しても影響を及ぼし，対象者のQOL（生活の質）を著しく低下させることが多い．わが国の理学療法士は，これまで主に急性痛に対する物理療法や徒手療法などの医学的アプローチを発展させて来たが，慢性痛に対する包括的理学療法の導入に関しては，欧米に比べてかなり遅れをとっているというのが実感である．教育的アプローチである腰痛教室は国内でも歴史があり，標準化の域に達しているといえるが，それ以外では学際的疼痛アプローチを導入したごく一部の施設で実践されているに過ぎない^2）．

　そこで本稿では，慢性痛に対する包括的理学療法に焦点を絞り，その鍵となる概念である「パーソナル・コントロール」と「行動随伴性」について解説した上で，包括的理学療法の方法論とその具体的な事例を紹介する．さらに，欧米で行われている慢性腰痛症に対する包括的理学療法として，「行動理論に基づいた段階的活動プログラム」を紹介する．
