健康増進・内部障害に対する理学療法学教育―現状と課題

わが国では近年，メタボリック症候群や内部障害者が増加している．今後，患者数，医療費のさらなる急激な増大が予測されており，健康増進・内部障害に対する理学療法は，社会的ニーズに合致した重要な分野である．すでに，疾病に起因する内部障害で多数を占める呼吸器疾患・呼吸障害，心疾患や末梢血管疾患に対する運動療法は，国際的な各種の診療ガイドライン1～3）において重要なエビデンスとして位置付けられており，関連する国内学会においてもステートメントや診療ガイドラインを作成している4,5）．また，これらに対応し，診療報酬も改定されつつある．しかし，現状では理学療法学教育はまだ社会のニーズに十分対応しているとは言えず，またオン・ザ・ジョブトレーニング（on the job training）が不足したまま養成校の教員になっているというリハビリテーション専門医からの指摘もある6）．そこで，本稿ではヘルスプロモーション，健康増進・内部障害に対する理学療法学教育について，その位置付け，世界での取り組みと教育課程，日米の理学療法学教育の比較研究を紹介し，これらを通じてわが国における理学療法学教育の現状と課題を提示する．
