地域在住高齢者におけるtrail walking testの信頼性および妥当性の検討

要旨：本研究の目的は，われわれが考案したtrail walking test（TWT）の信頼性と妥当性について検討することである．TWTは，注意機能検査であるtrail making test part A（TMT part A）を，床面上で自身が移動しながら行うものであり，5×5mのエリア内に，①～⑮までの番号のついた15本の旗をランダムに設置し，①から順に⑮まで通過していき，それに要した時間を計測する．特定高齢者30名を対象とし，1回目のTWT測定より1週間後に再測定を行い，信頼性の検討を行ったところ，級内相関係数が0.953と良好な再現性を示した．次に基準関連妥当性の検討として，TWTと他の指標との相関関係を検討したところ，timed up and go test，TMT part Aと中等度の相関関係を認め，バランス能力，総合的下肢機能，注意機能などの側面を評価しうることが示唆された．TWTによって転倒と非転倒を判別することができるのか判別分析を行ったところ，71.72秒でカットオフ値となり，この正当性は82.4％であった．これらのことより，TWTは信頼性が高く，注意機能的側面，身体機能的側面の両者を評価しうる新たな総合的指標であり，日常で用いる複合的機能を捉えることが可能な転倒リスク評価として有用であると考えられた．
