脳性まひの理学療法介入におけるクリニカルリーズニング

1940年代から1950年代にかけて，Salkワクチンによりポリオ患者の発生率は激減し，理学療法士の関心はポリオから脳性まひなどの神経学的障害をもつ患者に移っていった．NDT（神経発達学的治療法）や感覚統合療法，Temple Fay，Kabat，Rood，Vojtaらの治療法は神経生理学的アプローチに分類される．これらの治療法は，運動発達領域におけるMcGraw1）とGesell2）が提唱した神経成熟理論から引き出された仮説を基盤としているが，その仮説には根拠が明白なものと，不明確なもの（暗黙の仮説）が存在する．この神経成熟理論からすると，異常な運動制御は正常な反射-制御機構の崩壊によるものと考えられる．そして，中枢神経系の高位レベルにおける障害は，下位中枢の制御不足や解放につながるとし，その結果，原始反射が運動行動を支配すると捉えている．このような運動制御の見方が神経生理学的アプローチの基礎となっている．また，神経成熟理論では，運動発達は中枢神経系の成熟に依存するとされているため，発達モデルは一定の順序で階層的かつ直線的な変化を示すと考えられている．この発達モデルは，脳性まひ児の治療に当たっている小児領域の理学療法士におけるクリニカルリーズニング（以下，臨床推論）の原理となっている．
