はじめに

　様々な症状を示すパーキンソン病患者において，われわれ理学療法士が，臨床上よくみかける歩行障害の1つに，すくみ足がある．すくみ足とは，歩行能力を有するにもかかわらず，何秒かにわたって歩行ができなくなったり，動き始めることができなくなったりする状態のことをいい^1），日常生活動作を大きく阻害する．また，バランスを損なわせるため，パーキンソン病患者における転倒要因の1つとしても考えられている^2）．したがって，すくみ足に対して理学療法が担う役割は大きく，すくみ足の病態を理解した上で理学療法を実施しなければならない．本稿では，すくみ足の病態を概説し，すくみ足に対する理学療法アプローチについて自験例を交えて解説する．
