パーキンソン病患者における呼吸障害と拘束性換気障害

　緩徐進行性の疾患であるパーキンソン病（Parkinson's disease）患者において，呼吸障害や誤嚥性肺炎などの合併症は，日常生活を障害する主要な因子となるとともに，生命予後，機能的予後に影響を与える^1,2）．また，パーキンソン病患者における呼吸と嚥下の協調性不全は，誤嚥性肺炎の発症に関与している^3）．

　インダクタンスプレチスモグラフィー（inductance plethysmography）法を用い，胸腹部の運動量と呼吸量を同時に測定した研究では，パーキンソン病患者では健常者と比べて％肺活量（％VC），％努力性肺活量（％FVC），胸部運動量，腹部運動量が有意に低下していた．また，％VCと胸部運動量の間には有意な相関があり，パーキンソン病患者では，胸壁のコンプライアンスが低下することで呼吸量が制限されていることが示唆された^4,5）．このように，拘束性換気障害を有する患者に対しては適切な評価と治療が必要である．
