NICUにおける呼吸理学療法ガイドライン

はじめに

　診療ガイドライン（clinical practice guideline）とは，「医療者と患者が特定の臨床状況で適切な決断を下せるよう支援する目的で，体系的な方法に則って作成された文書」である．1998年度に厚生省は，国内の診療ガイドラインを作成することを推進した．その後，いろいろな学会において診療ガイドラインが作成されてきた．ガイドラインはevidence-based medicineに基づいたものであるべきで，ランダム化比較試験の結果から得られるエビデンスを吟味・評価し，その結果に基づいてどんな治療をすべきか，すべきでないかなどを勧告する必要がある．そのため過去のガイドラインの多くが著名な専門家の意見交換や経験によって作成されていたのに比べ，信頼性は高い．しかし，診療ガイドラインを活用する際に注意すべきことは，ガイドラインはあくまでも標準的な指針で，すべての患者に画一的な診療を強制するものではないということである．

　適切に診療ガイドラインを評価するために，ガイドラインの研究・評価用チェックリスト〔Appraisal of Guidelines for Research & Evaluation（AGREE）instrument〕や，日本医療機能評価機構による「Minds診療ガイドライン作成の手引き2007」が用いられる．AGREEは①対象と目的，②利害関係者の参加，③作成の厳密さ，④明確さと提示の仕方，⑤適用可能性，⑥編集の独立性の6つの観点が合計23項目のチェックリストになっている．また，IOM（Institute of Medicine）による診療ガイドラインの評価項目には，①妥当性，②信頼性/再現性，③適用可能性，④柔軟性，⑤明確性，⑥関係諸団体の参加，⑦定期的な再検討，⑧作成方法の明示の8項目を挙げている．

　ここでは，2003年に日本未熟児新生児学会で発表したNICUにおける呼吸理学療法のガイドライン第1報と，2009年の改訂版（第2報）について概説する．
