膝関節鏡視下手術の進歩と位置づけ

1．関節鏡とは

　関節鏡とは，関節内を観察するための光学機器であり，膝関節の場合は，皮膚切開部（約1cmの小切開部）から関節鏡（径4.0mmのカメラ）を挿入し，生理食塩水で満たした関節内を観察する．別の小切開部より様々な手術器具を挿入することにより，切除や縫合といった治療手技を組み合わせることが可能である（図1）．このように，膝関節鏡は検査手技と治療手技の両方の役割を担っているといえる．

　関節鏡の発達は膝関節外科に飛躍的な進歩をもたらし，手術手技の進歩と新しい関節鏡器具の開発によって，その適応は今なお広がっている．
