変形性膝関節症患者の日常生活動作に対する2種類のサポーターの効果比較

要旨：膝OA患者においてACLとMCLをパッドと蝶番付きの支柱によって補強する機能を有する比較的長いサポーターと，ネオプレーンゴム製のベルトに側方動揺性を防ぐコイルを縫いこんだ比較的短いサポーターを装着した場合での日常生活動作における疼痛の改善度を比較し，各装具の適応を考察した．

　62例の膝OA患者を長いサポーター装着群（31例）と，短いサポーター装着群（31例）に無作為に振り分け，装着2週間前後での10項目の日常生活動作における疼痛の改善度を比較した．その結果，でこぼこ道での歩行における膝疼痛に関しては短いサポーター群のほうが有意に優れていた（P＝0.014）．その他の9項目に両群間の有意差はなかった．その理由として，ACLを補強するパッドが付随していると，でこぼこ道歩行時などの不安定な状態では重心動揺性がかえって増加するため疼痛が引き起こされると考察した．このことから，膝OAに対する軟性装具療法ではシンプルな装具でもスポーツ外傷で用いるような強固な固定力を有する装具と同等の効果（一部ではより高い効果）が得られると結論した．
