変形性股関節症のバイオメカニクスとADL指導

はじめに

　総務省統計局の人口推計2010年9月報では，2010年4月での日本人人口は約1億2千579万人で，このうち65歳以上の高齢者は23.2％にのぼるという確定値が示されている^1）．人口の高齢化率の上昇は，多くの高齢者が加齢に伴う運動器の機能低下と器質的変化を起こし，変形性股関節（osteoarthritis of the hip joint：股OA）をはじめとする，多くの下肢関節の疾患を増加させることは想像するに難くない．また股OAの患者は，ほとんど全例といってよいほど理学療法の対象となる．現在のところわが国での股OA罹患者率は欧米諸国と比べ高くはないものの，今後は上記のような人口動態のもたらす因子と，生活様式や食生活の欧米化によって罹患率が上昇すると考えられ，理学療法サービスの対象として重要な疾患であると思われる．

　本稿では，股関節に求められる機能をバイオメカニクスの観点から解説するほか，最近の股関節機能に関わる新しい知見のいくつかと，変形性膝関節症でみられる脚長差に対し，臨床でよく行われる補高の考え方と注意点，歩行パターンとエネルギー消費について，日常生活活動（activities of daily living：ADL）の指導を含めて記述する．
