神経筋疾患に対する筋力増強

はじめに

　神経筋疾患のなかには筋ジストロフィー，多発性筋炎，ギラン・バレー症候群（Guillain-Barré syndrome），筋萎縮性側索硬化症（amyotrophic lateral sclerosis：以下，ALS）など多くの疾患が含まれる．これらの疾患では，ほぼ共通して筋力低下を伴い，運動療法においてはその回復が主要な目標となることが多い．各疾患にみられる筋力低下は，その疾患の病態により生じているため，病態の悪化・改善により筋力が低下・増強する．そのため，病態が大きく異なるこれらの疾患では，筋力増強の過程も様々である．

　神経筋疾患の筋力増強を考える場合，ほぼ共通して問題となるのが過用性筋力低下（overwork weakness）や高い疲労性である．過用性筋力低下は，一定期間の筋の使用によって生じた筋力と持久力の低下が持続する状態である^1）．疲労は，生理学的・心理学的両面の意味をもつ多次元的な概念である．そのなかでも生理学的疲労とは運動の持続により起こる筋機能の一時的な機能不全である^2）．そのため，神経筋疾患にみられる過用性筋力低下と疲労とは異なる概念であるが，高い疲労性を示す時期に過用性筋力低下が生じやすいと考えられているためか，切り離して議論されることは少なく，関連性の高いものとして述べられることが多い．

　本稿では，神経筋疾患にみられる疲労の病態を述べたうえで，過用性筋力低下を考慮した神経筋疾患患者の筋力増強練習について概説する．
