呼吸障害の理学療法スタンダード

はじめに

　わが国の呼吸理学療法に関する“専門用語と手技”の統一を目指した画期的な書に「呼吸理学療法標準手技」^1）がある．このなかで「呼吸理学療法」は「呼吸障害に対する理学療法の呼称および略称さらには総称であり，呼吸障害の予防と治療のために適応される理学療法の手段」と定義されている．また，「換気の改善（換気量増大，不均等分布の是正，肺容量の増大など），気道内に貯留する分泌物の誘導排出，胸郭運動性の増大，酸素化の改善，自覚症状改善などである．最終的には，各種の呼吸障害によって引き起こされる日常生活活動制限の予防あるいは改善，拡大であり，早期離床や運動耐容能の改善も重要な目的である」と述べられている．

　本稿では「呼吸理学療法標準手技」出版後，発表された2つの診療ガイドライン^2,3）を参考に「スタンダード」と位置づけ構成した．なお，引用した論文で採用されているエビデンスレベルはそれぞれ異なるので，表1のように統一して表記した．
