大腿骨頸部骨折の理学療法スタンダード

疾患概要と整形外科的介入法

　理学療法士が介入する高齢者の骨折で，大腿骨頸部骨折は代表的なもののひとつである．わが国の大腿部骨折および転子部骨折の年間発生率は40歳から年齢とともに増加し，2002年の全国調査では約92,400名，2010年では170,000名，2030年には260,000名に達すると推定されている^1）．大腿部骨折および転子部骨折は社会的にも医療経済的にも及ぼす影響が大きく，高齢者に対するリハビリテーションおよび予防の観点から積極的に取り組まなければならない課題のひとつである．

　大腿骨頸部骨折は大腿骨頸部内側骨折ともいい，関節包内骨折のため骨癒合率が低く治癒しにくい骨折の代表である．観血的治療が積極的に行われ，骨折のタイプ，年齢，患者の全身状態より，sliding hip screw（SHS）法などの骨接合術や人工骨頭置換術などの人工物置換術が選択され施行される．本稿では特に人工骨頭置換術の「根拠に基づいた理学療法」の評価と治療について概説する．
