がん患者の心の問題とリハビリテーションへの期待―サイコオンコロジーの知見から

はじめに

　がん診断と治療の進歩により，治癒率や生存率は上昇したが，それでもわが国におけるがんによる死亡は年間30万人を超え，死亡原因の第1位でもある．“がん＝死”のイメージは未だに浸透しており，がんの病名告知からはじまる闘病生活は多くのストレスに曝されることとなる．その結果，不安，抑うつを呈する患者は多く，希死念慮や自殺企図といった問題に波及することも少なくない．介護者である家族が不安，抑うつ症状を訴えることもある．また，病状の進行や治療の過程の中で，患者がせん妄と呼ばれる精神症状を呈することもある．そのようながん患者や家族の精神・心理的問題を取り扱う領域がサイコオンコロジーであり，サイコオンコロジストは，がん罹患によって発生した精神・心理的苦痛を軽減する役割を担っている．サイコオンコロジストが提供する医療は直接がんそのものを治癒するための治療ではないが，がん患者の抱える障害の軽減を図るという点で，リハビリテーションと類似したアプローチともいえるだろう．本稿では，リハビリテーションへの動機付けにも影響を及ぼすがん患者の精神・心理的問題を取り上げ概説した．また，サイコオンコロジストの立場からリハビリテーションに期待することも触れさせて頂いた．リハビリテーションの現場で参考にしていただければ幸いである．
