慢性炎症と生活習慣病

はじめに

　近年，心筋梗塞や脳梗塞などの動脈硬化性疾患が増加傾向にある．これらの上流に位置する生活習慣病の治療として，生活習慣の改善や薬物療法が重要視されている．一方，これらの動脈硬化の発症・進展において「炎症」というキーワードが注目されている．

　元来，動脈硬化の原因についてはコレステロール血漿成分の滲出に基づいた脂質蓄積説が主流であった．しかし，1976年にRossら^1）によって血小板由来増殖因子（platelet-derived growth factor：PDGF）による血管内皮の傷害反応説（response-to-injury hypothesis）が提唱された．その後，マクロファージやサイトカインなどを軸とした炎症反応を重視した説に修正され，それ以降の動脈硬化のテーマは徐々に「炎症」へとシフトしていくこととなった．

　動脈硬化の成因論として「炎症」が強く関与していることは実験的にも臨床的にも支持されており，その正当性が検証されている．本稿では，動脈硬化を炎症性疾患としてとらえ，動脈硬化の発症，進展と「炎症」の関与を概説することで動脈硬化予防の理解を深めることにつながれば幸いである．
