3．糖尿病足病変

はじめに

　糖尿病細小血管障害や大血管障害が複雑に絡み合って発症する糖尿病足病変は，末期に至らなければ発症しないという認識から糖尿病患者にとって現実感の薄い存在である．さらに，神経障害による知覚消失が病態に深く関わる以上，患者が足病変を自覚するのは難しい．したがって，医療機関へ受診する際には，足壊疽や全身の発熱など深く進行した状態になっていることが多い．これらの病態を考えれば，糖尿病を合併症としてもつ症例と関わった場合，積極的にリスクを評価すべきである．本稿で述べるように，理学療法士は足病変管理という点で貢献することが可能である．多くの診療科の医師や糖尿病認定看護師などとチームを組み，糖尿病足病変の治療や予防に積極的に関与すべきである．

　一方で，多くの臨床現場では，他疾患に対する理学療法を行う際に，合併症として糖尿病足病変に出会うことが一般的であろう．合併症として糖尿病が存在する症例，切断症例，足病変既往症例，透析症例などその状況は多彩である．現状では足病変リスクや疾病状況が一人ひとり違う患者に対し，どのような評価，介入を行うべきか不明確である．そこで，本稿では対象者の疾患や状況ごとに段階を追って介入内容を整理することを試みる．
