脳卒中早期（急性期）の理学療法スタンダード

近年，医療の質の確保や治療の標準化の重要性が多く唱えられ，エビデンスに基づく治療体制を整備していくことが求められている．理学療法（以下，PT）においても同様に，evidence-based physical therapy（EBPT）の手法を利用して，個々の患者によりよいPTを提供するための方法を用いることが望まれている．脳卒中では，2009年に「脳卒中治療ガイドライン2009」1）が刊行され，科学的根拠に基づき効果があるとされる情報を取り入れることが比較的容易に可能となってきた．また，米国心臓協会/米国脳卒中協会（AHA/ASA）ガイドライン2），欧州脳卒中機構（ESO）ガイドライン3）が発表され，世界的にも脳卒中の治療はエビデンスに基づき標準化されたものが提供していけるよう整いつつある．また，そのなかで脳卒中急性期リハビリテーション（以下，リハ）の必要性は共通して述べられているが，これらの内容のすべてがEBPTとして臨床に即活かせるとは限らない．
本稿では，脳卒中急性期PTにおける，基本的で不可欠な評価と治療について具体的に述べていきたい．
