関節リウマチの最新治療

　2003年に本邦で初めての関節リウマチ（rheumatoid arthritis：RA）に対する生物学的製剤が承認されてから間もなく10年を迎える．生物学的製剤は，それまでの治療と比較して著しく強力な効果を持ち，RAの薬物治療にパラダイムシフト（革命的かつ非連続的変化）をもたらした．RAが不治の難病から，薬で治療可能な疾患へと大きく変容を遂げたことにより，診断基準，評価方法，治療目標，治療体系，さらにはリハビリテーション（以下，リハ）までもが一変することとなった．

　RAの最新治療戦略は，発症早期に確定診断を行い，疾患活動性を客観的に評価し，予後不良が予想される場合に，遅滞なく生物学的製剤を含む薬物療法を体系的かつ積極的に開始して，臨床的，構造的，機能的寛解のすべてを満たした完全寛解を速やかに導入することである．さらに，薬物療法の効果を最大限に発揮させ，生涯にわたって障害を来さないためには，早期からの患者教育とリハによる関節保護をチーム医療で行う必要がある．本稿では，高い生活の質（quality of life：QOL）をめざした生物学的製剤時代のRA治療について概略する．
