腰椎椎間板ヘルニアの理学療法診療ガイドライン

腰痛に関しては，1994年に米国で最初のガイドラインが出されてから，現在14か国17のガイドラインが示されている．特に本邦では2001年に「科学的根拠（Evidence Based Medicine：EBM）に基づいた腰痛診療のガイドラインの策定に関する研究」1）が発表され，2011年に「腰椎椎間板ヘルニア診療ガイドライン（改訂第2版）」2），2012年に新たに「腰痛診療ガイドライン」3）が作成されている．
この理由には，厚生労働省の国民生活基礎調査有訴受診率（健康保険を使用しての受診者数）で腰痛が男女合わせて10年以上第1位となっているという誇れない結果が続いたことが挙げられる．腰痛を訴える患者数が多く受診する診療科が多岐にわたることを考慮して，診療ガイドラインを作成することによって内科医やプライマリケア医にも腰痛診療を可能にし，腰痛に悩む患者を科学的根拠に基づいて少しでも減少させることが目的とされている2～4）．
今回の理学療法診療ガイドラインでは，腰痛に関する多くの報告の中からまず疫学に関するエビデンスを整理し，その後保存療法と手術療法の比較に関する検討を加え，保存療法に必要な診断および評価に関して吟味した．言うまでもなく，腰痛とは1つの疾患単位ではなく症状の名称である．画像診断により確定できる腰痛は10～20％と言われ，80～90％は非特異的腰痛とされている5）．
本講座では，腰痛の中で特異的疾患の1つである腰椎椎間板ヘルニアに関して，エビデンスに基づき現状での理学療法評価・治療・介入に関して概説する．
