外傷性頸髄損傷患者の損傷直後からの下肢機能の経過分析

要旨：完全麻痺を呈した頸髄損傷者の受傷6週までの下肢機能の短期経過を検討した．急性期加療を行った外傷性頸髄損傷者19例（Frankel-A/B）に対し，骨傷の有無で分け，受傷6週までの各週における運動と感覚の状態を改良Frankel分類とInternational Standards for Neurological and Functional Classification of Spinal Cord Injury（ISCSCI）分類にて評価し，受傷6週時に下肢回復群と非回復群に分類した．骨傷者は全例受傷6週時においても著しい変化を認めず，非骨傷者11例中6例がFrankel-C/Dへ回復し，歩行動作を獲得した例もみられた．また，受傷1週のISCSCI分類の運動，触覚および痛覚スコアは，非回復群に比べ回復群が高値である傾向を示し，受傷6週までにおけるISCSCI分類の運動と痛覚スコアの変化量も回復群が高値を示した．非骨傷性頸髄損傷患者のなかで，受傷1週時の表在感覚が残存し，かつ受傷3週までに痛覚の回復を認めた症例は，歩行能力の回復を期待できる可能性が考えられる．
