小児心疾患

はじめに

　小児の心疾患は，先天性と後天性とに大別されるが，その大半は先天性心疾患（congenital heart disease：CHD）である．近年ではCHDに対する診断および内科的・外科的治療が長足の進歩を遂げ，「根治」と呼べる状態にまで回復した患者が多数存在するようになった．しかし一方で，さまざまな程度の心不全・チアノーゼ等を残す症例に加え，厳しい治療の過程で種々の神経学的・整形外科的障害などを負う子供も多く，「心臓の形に関しては治ったけれど，障害児としてさまざまな困難を抱えた患者」となったケースもしばしば経験される．手術不適応例の子供，後天性心疾患を患った子供も含め，理学療法を中心としたリハビリテーションにより生活機能の再獲得を必要とする症例は年々増加しつつある．

　本稿ではまず，わかりづらいと思われることの多いCHDを中心とした小児心疾患の代表的なものをピックアップしてその病態・診断・治療を概説し，次いで理学療法アプローチの際のポイント・留意点，課題や展望等について考按する．
